Community Living Support

Supports and assistance promoting independence and integration into a person’s community of choice, including acquiring the skills to make full inclusion happen. This can include a variety of activities such as budgeting, exercise, shopping, household care and taking part in community activities. This service is for those who do not receive residential supports.

The utilization of evidenced based and best practices in behavioral techniques, interventions, and methods to assist a participant with significant, intensive challenges that interfere with activities of daily living, social interaction, or work. Positive behavior supports are clinically sound and based on person-centered values considering what is important for the participant.

Person-Centered Coaching

Working under the direction of a positive behavior support specialist, this service is utilized when a barrier challenges the success of a participant in achieving the participant’s goals. This is a supplemental support provided to SCL-II waiver participants who utilize behavior supports and feel the need for extra focus on their person-centered behavior plan.

Assistance with eating, bathing, dressing, personal hygiene, and other activities of daily living as needed to promote full inclusion in the person’s community of choice.

Designed to provide an opportunity for a person to connect and become involved with clubs, associations and any other groups in the community. This includes recreational, educational, religious, civic and volunteer opportunities with an outcome of less reliance on formal supports and an emphasis on the development of personal social networks, membership opportunities, and natural supports for the person.

Personal Assistance

Enable a person to accomplish tasks that the personal normally would do themselves if they did not have a disability. This includes hands-on assistance, reminding, observing, guiding or training during an activity, assistance in managing medical care, and transportation to access community services and appointments.
